Good Practise in Hospital Discharge for supported people and unpaid carers

A workshop to explore the hospital discharge process, what good practise involves and people's rights when their discharge is delayed.
One of the key times that a person might need support to enable them to live well and safely at home, is when they are looking to leave hospital after a period of treatment. People may be needing support for the first time and find that their discharge from hospital is delayed as a result of waiting to get the right support in place.
This workshop will explore what is involved in the discharge process, what good practice looks like and the potential challenges that people face when looking to access the support that they need.
We will also look at supported people and unpaid carers rights during the discharge process and how they can address any issues they might come across.
In additon, we will consider good practise around discharge processes for people lacking the capacity to make decisions for themselves.
